Hockey Alberta requires all SPECIAL EVENTS, such as ID camps and Spring/Summer Camps be sanctioned.
Provide as much detail as possible regarding the camp or program you are planning to host in the Event Details below.
Please send a PDF of the camp event schedule to discipline@hockeyedmonton.ca as this will be required for the sanction request submission.
(This form MUST be submitted by any member MHA or registered program that wishes to host a Special Event recruiting new players or developing existing players outside of the traditional hockey season.)
- CONDITIONS -
Must have permission of Hockey Alberta before conducting such an event to be applicable for Hockey Canada Insurance Coverage. Sanction request must be submitted one (1) week prior to the event date.
The approved permit must be on site with the MHA/Program at all times during the event. The email containing confirmation of approval is the Permit.
If the camp involves games, copies of all Game Sheets and Officials' Reports must be sent to discipline@hockeyedmonton.ca within 48 hours of Game(s).
Copies of Injury Reports must be sent to the Hockey Alberta office within 48 hours of the conclusion of the event.
Event must adhere to Risk Management Guidelines as set out by Hockey Alberta.
