ALL discipline will be done by Sharlene (MDC RSC Edmonton). Game sheets and write ups are to be sent in PDF by email to discipline@hockeyedmonton.ca directly following the end of the game for review and suspensions.

I understand that I must turn in Game Sheets and Incident Reports to Hockey Edmonton within 24 hours of the end of the tournament. I also must turn in Game Sheets, Incident Reports, and any Respect the Game submissions to Hockey Edmonton Discipline immediately (discipline@hockeyedmonton.ca). I understand that failure to comply with the above will suspend future tournament privileges for my organization, and further disciplinary measures may be taken if I fail to comply with the above. I certify that the above information is true to the best of my knowledge and understanding. Submitting an inaccurate form will result in the cancellation of this and future sanction requests.
